Form AIMH-FRM-040 · v1.1

Referral form — fill it in your browser and send it here

Every copy gets its own reference number, printed at the bottom of both pages, so two referrals are never confused. Fill it on screen, sign it, and send it back without printing anything. Or fax it to (786) 388-0516.

Fill it online — nothing to download

Type straight into the form on screen, sign it, and we build the signed PDF for you. It works on a phone. This is the quickest way.

Fill the form online →

Prefer the PDF? The three steps below still work.

1 Get your numbered copy

Press the button. We issue a reference number, stamp it on both pages and the form downloads.

Plain copy, no number

Your reference number

Write it down. It is printed at the bottom of both pages and it is already filled in below.

Open this copy in a new tab to fill it →

  1. The form has downloaded. Find it in your Downloads folder.
  2. Open it, fill both pages on screen and save it.
  3. Then come back and press Send — below to send it from this page, or by email.

2 Fill it in on screen

The fields are typeable in Chrome, Edge, Firefox, Safari and Preview, and in Adobe Acrobat Reader. Complete both pages and save the file — in Chrome that is the download icon inside the PDF viewer, not the browser’s print button.

A referral is complete with the prescription, the diagnostic evaluation (or “evaluation needed” checked) and the insurance card. Send those in step 3 as well.

3 Sign it and send it

The signed FRM-040, the prescription or order, the diagnostic evaluation or CDE, the medical necessity letter, the insurance card front and back, the IEP, prior reports — whatever you have. PDF, JPG or PNG, up to 5 files of 4 MB each.

Two ways — use whichever suits you. Either one is enough.

✍️ Draw it here — mouse, trackpad or finger

📱 Or sign on your phone

Press the button to get your code.

Scan it with your phone camera, draw your signature with your finger and send it. It appears here on its own. The link lasts 15 minutes.

Signing records your name, the date and time with your time zone, this page, your browser and — only if you tick the box — your coordinates. Under the E-SIGN Act this signature has the same legal effect as one on paper.

Optional — you can send the form without agreeing to text messages.

Or send it by email

If your office prefers email, press the button: it opens your mail program with the address, the subject and the body already written. You then attach the PDF you saved, plus anything else.

This is what will be sent:

Email cannot attach the file for you — no website can. Attach the PDF you saved before you press send. If the address is not one of ours, we cannot guarantee the message reaches intake.

Online Referral Form

Complete the form below to refer a patient. Our intake team will follow up within 24–48 hours.

Optional — you can submit this form without agreeing to text messages.

What Happens After a Referral

Our intake process is designed to be simple and stress-free for families.

1

We Contact the Family

Our intake coordinator reaches out to the family within 24–48 hours to introduce our services, answer questions, and begin the onboarding process.

2

Insurance Verification

We verify the family's insurance benefits and handle all prior authorization paperwork so ABA therapy can be covered with no out-of-pocket costs where applicable.

3

Schedule Initial Assessment

Once authorized, we schedule a comprehensive behavioral assessment with a BCBA to develop an individualized treatment plan tailored to the child's needs.
